Rapid Covid Testing 

  • The DOH requires that we confirm all negative Rapid Antigen tests with a PCR test. If the employee/student has insurance, the PCR test will be covered. Formé will report all positive (+) tests to the DOH.
  • If the employee/student does not have insurance but has a valid driver’s license, NYS ID or active passport the PCR will be cover under the CARE Act.
